If you are splitting the cost of one room please complete 1 form per cardholder including the guests each cardholder is paying for. Under Special Requests please note the guest you will be rooming with.
You will be given an invoice upon confirmation that your deposit has been processed by the tour operator.

Credit Card Details
By providing credit card details, you are authorizing deposits and final payments to be applied on due dates. Please note all deposits and final payments are non-refundable and non-transferable.

Terms & Conditions
The travel agency, Preety Weddings & Travel, confirms arrangements with the noted supplier/tour operator for services such as airlines, hotels that may be included in your travel package above. We act on behalf of the agency to book services rendered from the above tour operator and as such, have no control over the personnel, equipment, operations, goods or services provided by the tour operator. The agency is a licensed business with Consumer Protection BC ensuring you are covered by any supplier bankruptcy. We work only with reputable travel providers but cannot claim nor assume responsibility or liability for any future changes, cancellations, force majeure in travel services provided above. Preety Weddings & Travel is paid based on commissions and fees, therefore not liable for anything more than the commissions and fees paid for your reservation. Preety Weddings & Travel has a fiduciary role to suppliers/tour operators as a travel agent. Preety Weddings & Travel is not liable for anything that the supplier/tour operator fails to do that causes damages, injury or loss including but not limited to accidents, unsafe, conditions, policies, bankrupt and defaults. Preety Weddings & Travel do not accept responsibility for lost wages, missed holiday time or any other additional expenses incurred as a result of changes in flight times. If the client before, during, or after travel falls ill, has any kind of injury, or subject any kind of loss or damage, Preety Weddings & Travel assumes no responsibility for any claim, loss, damage, cost or expense. It is the traveller’s responsibility to do their own research and to be aware of any and all advisories to the country you are traveling to and from. It is also the travellers responsibility to have of all documentation, up-to-date passports, visas, vaccines required to the country you are traveling to and from. If you have a criminal record, please seek legal advice regarding international travel, Preety Weddings & Travel are unable to provide specific guidance regarding entry requirements into various countries for those with criminal records Preety Weddings & Travel strongly encourages you to purchase travel insurance to provide you with coverage for unexpected developments of this nature. Nothing that Preety Weddings & Travel, including but not limited to our sub-agents, employees, or independent contractors) say to a client undercuts or undermines the terms and conditions.

Cardholder Agreement to authorize the deposit & final payment due

Client Agreement: I acknowledge that all payments are 100% non-refundable

Client Agreement: I agree that payments may be made direct to the selected tour operator on my behalf by Preety Weddings & Travel for travel arrangements for passengers named above and shall not decline, reject or challenge the identified amount. I agree not to file a dispute with the bank and/or credit card company for a chargeback. I accept that any disputed credit card charges is considered fraud, and I assume all responsibilities to reverse the chargeback claim, and I will pay the $150 chargeback supplier fee

Client Agreement: I acknowledge that travel insurance (including but not limited to cancellation, interruption & medical) has been recommended by Preety Weddings & Travel for this booking and I voluntarily take full responsibility for all claims

Client Agreement: I agree no refund of services will be provided when a passenger(s) has been denied boarding/entry to the country booked, due to inadequate documentation of the passenger(s). No refund of services will be provided when a passenger(s) is denied boarding of an aircraft due to personal conduct and behaviour. No refund of services will be provided when passenger(s) are refused entry to any country due to any past criminal record.

Client Agreement: On behalf of all persons named on this form, I, the card holder and lead traveller, agree that all information provided above is correct & matches the information on the passport(s) of the traveller(s) and agree to the terms & conditions. I understand that if any of the information I have provided is incorrect, I will be the sole responsible individual in correcting the information and that I will be solely responsible for paying any and all fees associated with the correction of any passenger names and/or data. I am aware of all advisories for the country I am traveling to & from and have done my research if there are any risks, high crime, civil unrest etc. I, the client, release Preety Weddings & Travel from any & all claims related to the pandemic/force majeure, illness, accidental death, lost wages/missed vacation time, ordinary negligence including but limited to bad behavior, over consumption of alcohol/drugs, crime, risks, intentional acts, or damages related to this booking and will cover all defense costs due to the agency (Preety Weddings & Travel) if sued. I confirm I have read the terms & conditions listed on this form and agree that I am bound by them:
